Doctors from the Clinic of Surgery and the Department of General and Clinical Pathology at the “Queen Joanna Hospital – ISUL” hospital in Sofia have found a dangerous complication following recovery from Covid infecion, the medical institution announced on January 27.

The post-Covid condition is simultaneous thrombosis (occlusion) of both venous and arterial vessels.

The complication was observed in three emergency patients treated at the hospital last year. These are three men aged between 63 and 78 who had been ill with Covid-19 between 5 and 7 months before surgery.

The first patient, aged 78, was admitted to ISUL with severe abdominal pain and vomiting.

After imaging and blood tests, he was diagnosed with acute abdomen and emergency surgery was scheduled. During the operation it became clear that the man had peritonitis of the colon.

"During the operation, I was impressed by the atypical appearance of the colon, which was completely white and not supplied with blood," said Dr. Dimitar Peichinov, one of the most experienced surgeons at the ISUL Surgery Clinic.

The second patient, aged 63 (with underlying health conditions including non-insulin-dependent diabetes mellitus and hypertension), was brought to the Emergency Department by an ambulance team with evidence of abdominal pain and impaired general condition.

Instrumental and blood tests were performed. The doctors observed increased size of the spleen without the man having any evidence of trauma in this area. During the emergency operation to remove the spleen, surgeons again encounter something unusual: severe ischemia, white tissue and abscess formed.

The third patient, aged 66, had concomitant non-insulin-dependent diabetes, gout, and overweight.

The patient was admitted to the emergency with evidence of acute gastric bleeding after polypectomy (removal of polyps). During the operation, the surgeons again came across a similar finding: organs and tissues deprived of blood supply, which are difficult to manipulate.

The conclusion of Dr. Ivan Terziev, Head of the Department of General and Clinical Pathology at ISUL hospital from the materials sent to him is categorical: simultaneous arterial and venous thrombosis, and stagnation in microcirculation, without serious inflammatory reaction.

According to Dr. Terziev, who is one of the most experienced pathologists in Bulgaria, the damages to the walls of arterial and venous vessels is a result of coronavirus.

Dr. Terziev and Dr. Peichinov decided to review the medical records of the three patients once again. They found that in addition to all three having had the coronavirus, they went through the infection with severe symptoms and stayed in hospital for a long time.

The other fact that impressed the doctors was that all three have not been vaccinated.

His advice to patients with underlying heart disease is, in the first place, to get vaccinated, and those who have failed to do so and have had Covid-19 with severe symptoms to have regular preventive checks.

His other appeal is that people should not overdo salicylates (Aspirin), because only last day he had three cases of bleeding ulcers due to long-term use of these medicines.

Dr. Terziev and Dr. Peichinov are about to publish the data from their observations in a specialized medical journal.
